NDIS service agreement via online sign up
I agree to enter into this Service Agreement as a participant in the National Disability Insurance Scheme (Participant), and Trugrade Medical Supplies, ABN 19 476 823 124 (Provider)
- This Service Agreement will commence on the day this form is submitted and remain in place for the duration of the current NDIS plan held by the participant (the NDIS Plan End date)
- The Service agreement will continue when the Participant receives a new NDIS plan and wishes to continue purchasing medical consumables from Trugrade Medical Supplies
- This Service Agreement is made for the purpose of providing Medical Consumables that the Participant has approved under the approved NDIS plan.
- The Service agreement can be terminated by either party in writing by providing 14 day's notice period. All obligations entered by either party prior to the plan termination notice will be the responsibility of the respective party to be completed as of the date of plan termination.
- The Participant confirms that if the Consumables supplied by the Provider on order by the Participant, are unable to be claimed from and paid by the NDIA and / or disapproved by the Plan manager or the NDIA, the Participant will be personally liable for the payment of the Invoice amount. In such an event, the Provider will notify the Participant by email with the Invoice details and the payment will be completed by the Participant within 3 days of such notification.
- If any of the information supplied on the form above is incorrect or cannot be validated by the Provider, it will be deemed that No service agreement has been created or entered into by the Provider with the Participant.
